The Partners for Healthy Families (PHF) program is an early childhood home visitation program that builds on the best of Early Head Start and Parents As Teachers programs. PHF serves at-risk families with children under the age of five.
In Grundy Count,k PHF focuses on children under the age of three (including prenatal) who do not qualify for Early Head Start. Operation Threshold has provided PHF in Bremer, Butler, Franklin and Grundy counties for over five years (the first three years as Parents As Teachers). Utilizing thePartners for a Healthy Baby curriculum, the PHF home visitors conduct weekly, or bi-weekly, 60 – 90 minute home meetings with each family (depending on each families’ needs). During these visits, the home visitor shares age-appropriate child development information with parents, helps parents learn to observe their child’s development, addresses parenting concerns, and engages the families in activities that provide meaningful parent/child interaction and support the child’s development. Overall, The PHF program provides families with home visits, child development screenings for children, and an active resource network for participants.
